    As in this comic strip: 

The doctor has never seen something that bad in the past until just now. You see? 
It's a past related to the present.

3. TIME EXPRESSIONS WITH PRESENT PERFECT

3. PRESENT PERFECT OR PAST SIMPLE

 As you can see, the past simple is used for finished actions 
that happend in the past. We often use time words like yesterday, 
a few years ago, last week etc. these actions aren't connected 
to the present.
